8.1q Provider Statement

This form is for establishments to send to providers to ask them to declare that they have appropriate arrangements in place.  Please do not send this form to providers which hold an LOtC Quality Badge, unless you are seeking confirmation of the questions in section A that are not covered by the Quality Badge.

If you make any changes to the form, you must remove the OEAP and National Guidance logos, but acknowledge OEAP National Guidance as the source using words such as “this form is based on a model form published in OEAP National Guidance at oeapng.info“.
